Case Management provides personalized guidance and support for families navigating kinship care, adoption, guardianship, and other non-parental care situations. Families work alongside a Family Advocate to identify goals, overcome barriers, and connect with resources that strengthen stability and well-being.

Every family's situation is different. Case Management helps families navigate challenges, identify goals, and access the support needed to create safe, stable, and nurturing environments for children.
Through one-on-one support from a Family Advocate, families receive individualized guidance based on their specific needs and circumstances.
Kinship caregivers often step into their role unexpectedly. Family Advocates help caregivers understand available resources, navigate legal and financial systems, access benefits, and connect with community supports.
Families who have achieved permanency may still encounter challenges related to trauma, education, behavioral concerns, or family adjustment. Case Management provides ongoing support and resource navigation.
Our goal is to help families build long-term stability while ensuring children remain connected to safe and supportive relationships.
